Mid Devon Healthcare is the Primary Care Network supporting Bow Medical Practice, Mid Devon Medical Practice, Redlands Primary Care and Wallingbrook Health Group.
What is a PCN?
To meet the needs of those living with long term conditions such as diabetes and heart disease or suffering with mental health issues, practices have begun working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in primary care networks.
The Mid Devon Healthcare PCN's vision is to work together to achieve sustainable high-quality general practice and to maintain and improve local services for the population it serves.
